For testing orders please submit both the order form and the Applicant List template.
Note: You will want to complete the Applicant List template before selecting and completing an order form below. Save the template and close. (To attach to an e-mail the template must be closed.)
Forms can be submitted via E-mail or fax.
Step 1: Select the correct order form.
Step 2: Fill out required information.
Step 3: When the form is complete you can print a copy of the form for your records and click 'Submit' or click 'Submit' without printing a copy. After clicking on the 'Submit' button your order form will appear as an attachment in an E-mail Composition Window.
Step 4: If using Outlook select Insert from the drop down menu in this window and then select File. You will then need to find the location of your applicant list template and click twice to attach.
If using another e-mail service provider use the options given for attaching a document.
Step 5: Select Send message.
